Our Integrated Care Team
Collaborative Practitioners. Shared Purpose.
At PeakMD Health, we believe healing unfolds through collaboration—between disciplines, data, intuition, and human connection. The PeakMD Collective brings together independent practitioners in massage therapy, structural integration, fitness training, energy medicine, and coaching. Each shares a common purpose: to help our patients restore balance, build resilience, and live in alignment with their North.

Working in concert with PeakMD Health, our Collective represents the living application of precision medicine—science guided by presence, physiology supported by touch, and measurable health reinforced by meaning. Together, we form an ecosystem of care that extends from metabolic optimization to soulful renewal.

Diane Flowers, LMT
Health Coach • Lead Daybreak Trainer • Retreat Curator
Diane is the lead health and training coach at Daybreak, where she guides clients through the Restore and Fortify programs with a focus on building strength, resilience, and long-term function.

Her work blends detailed anatomical understanding with real-world application. She has a unique ability to assess how the body is moving, identify limitations, and translate that into clear, progressive steps forward. Whether someone is rebuilding after injury, working through chronic pain, or ready to train with more intention, Diane meets them where they are and helps them move forward with confidence.

Diane’s approach bridges the gap between physical therapy and traditional fitness. She integrates movement, recovery, nutrition, and habit formation to help clients not only restore function, but build durable, sustainable strength.
As PeakMD Health’s lead health coach, she also works across all pathways—helping patients translate clinical insight into daily practice and long-term consistency.
Beyond the clinic, Diane curates immersive retreat experiences that combine movement, mindfulness, and nature, creating space for both restoration and growth.
